Information to Bring to the Arrangement Conference

  • Full legal name
  • Date and place of birth
  • Current address
  • Marital status
  • Spouse’s full name (including maiden name, if applicable)
  • Mother’s full maiden name
  • Father’s full name
  • Social Security number
  • Education: Highest level completed (elementary, high school, college, or graduate school)
  • Occupation and employer
  • Place of birth for parents (if known)
  • Life Insurance: Company name and policy number (if applicable)
  • Memberships: Religious or church affiliations, fraternal or service organizations, union memberships, or special recognitions
  • Copy of DD214 (discharge papers)
  • Branch and rank
  • Dates of service (enlistment and discharge)
  • War or conflict(s) served in
  • Military honors or medals (if applicable)
  • Preferred clergy or celebrant
  • Names of active and honorary pallbearers
  • Music, readings, or scripture selections
  • Preferred clothing or jewelry
  • Burial property information (cemetery name, section, and plot, if already owned)
